I wonder what the meaning is of Specific restraint form record - is it "record in a specific form" or "(leave a) specific record in a form"?
Are the doctors asked to "leave a record in a specific restraint form" or to "leave a record (in the patient's medical history) on the specific form of restraint used on the patient"?
The more I try to understand this little phrase, the more confused I get.
